The system logged her as inactive for eleven minutes. Those were the eleven minutes she spent deciding not to send the alert. She had learned, without anyone telling her, to move her mouse while she thought.

He had been away for four months. The project was in another city, the kind of distance that makes weekly trips home expensive and monthly ones almost impossible.

He packed the van himself, the way he always did. Tools loaded in the order he would need them. An address on a piece of paper on the passenger seat. He had been doing this for eleven years. No crew. No supervisor. That was the arrangement.

The dust didn't look like much. It rose from the saw and settled on his clothes, his arms, the floor. He swept it up at the end of the day. He had swept up stone dust his whole working life. Nobody told him what made this dust different.

At six in the evening, on a platform in the North Sea, a maintenance job ran out of time. The permit was correctly filled in. Correctly filed. Never verbally handed over. 167 people died. The official inquiry named it plainly: failures in communication at shift handover.

He was cleaning gutters. That was the whole of the job that morning. He had been given the task, not the terrain. The fragile section of roof had been there before he arrived. Other people had walked past it. He stepped onto it.

The certificate proved he had reached the end. It proved nothing else. One tablet moving along a queue of forty workers before dawn, audio inaudible over the noise of the gate. He passed the quiz. The gate opened. The system said he was ready.

A driver pulled up in a heavy vehicle. He had been on the road for hours to get there. The gate stayed closed and the process continued exactly as it always did. He had paperwork to complete before he could pass. The gate took what was left of him before the day even started.

The most dangerous moment on a worksite is not the one nobody saw coming. It is the one the worker saw coming and said nothing about. I have felt it myself, not on a worksite, but in a meeting room, looking at a strategy I wasn't sure about, and staying quiet anyway.

The most important work never happened at my desk. It happened in hospital corridors, at site gates before dawn, in the offices of people navigating systems that assumed they already understood the rules.
